Assessment and Care Planning

These two course can be either be taken as two half days or as one whole day course.

These courses are available free of charge only to health and social care workers working in Surrey. To book a place on either or both courses, please either apply on-line  or  print off and complete the booking form to be found here.

Please note that the closing date for applications is one week before the date of the course.

  • Setting goals
