JOB TITLE: Patient Admission & Discharge Coordinator
REPORTS TO: Ward Sister
Key Working Relationships: All members of the Multi-Professional Team
JOB SUMMARY
To undertake the effective coordination of the hospice patient admission and discharge systems to ensure the delivery of a modern reactive patient focused service working in close collaboration with hospice clinicians, administrative staff, multi disciplinary team members and communicating with all relevant primary and secondary care services, patients and carers as required.
Willing to undertake work within the hospice / community settings visiting patients prior to admission. Key liaison person between home and hospice.
KEY DUTIES AND RESPONSIBILITIES
ADMISSION
1. Develop and maintain systems and pathways for all patient admissions to the hospice ensuring awareness of the hospice procedures are communicated to relevant parties
2. Ensure that processes are in place for all referrals including planned and emergency admissions of a patient to the hospice
3. Ensure that administrative processes are in place to confirm the accuracy and detail and receipt of referrals
4. Attend bed state meetings and manage the allocation of patients to available beds in close consultation with clinicians and Ward Sister
5. Coordinate the patient admission process in liaison with the MDT Co-ordinator, SCT and all other appropriate people
6. Liaise with Ward Clerk and SCT the system for the booking of transport for patients being admitted to the hospice ensuring timely effective liaison with internal and external parties.
7. Liaise with appropriate internal and external parties to ensure that any out of area referrals are appropriately authorised
8. Liaise re visits to hospice pre-admission in line with hospice policy
9. Manage respite calendar / patients in liaison with Ward Sister in line with hospice policy
10. To welcome and orientate patients and visitors to the IPU.
DISCHARGE
1. Develop and maintain systems and pathways for all patient discharges from the hospice - working with the whole multi professional team.
2. Ensure awareness of the hospice discharge policies are communicated to relevant parties
3. Liaise with the multi disciplinary team to coordinate the effective administration of the patient discharge process, including attending the MDT meetings
4. Liaise with the Ward Sister (or in her absence the relevant Senior Staff Nurse) on all relevant issues raised at discharge planning meetings
5. Liaise with ward team to ensure that the discharge process is fully coordinated and patients, carers and relevant internal and external staff are fully aware of the timescales and arrangements.
6. Coordinate and manage, in consultation with the Ward Clerk, the system for the booking of transport for patients being discharged from the hospice ensuring timely effective liaison with internal and external parties including CNS team.
7. Ensure that all relevant discharge information is accurately completed and processed on time
COMMUNICATION
1. Develop and maintain systems to ensure effective communication between the hospice multi disciplinary teams, community, hospital teams other primary care services and relevant external parties (e.g. district nurses, volunteer /support services, nursing homes, Marie Curie)
2. In cooperation with the hospice multi disciplinary teams and other relevant external bodies develop systems which promote and records user feedback
3. Promote an open culture which promotes a positive ‘can’ do attitude
4. Establish and maintain relationships with health and social care agencies / funding processes. To be a key link recognised by external agencies for all aspects of admission / discharge.
ORGANISATIONAL
1. In liaison with clinicians and ward mangers act as a key link for admission and discharge coordination issues within the hospice
2. Liaise with senior management and multi disciplinary teams as required to ensure the hospice meets the requirements for the Healthcare Commission
3. Attend relevant meetings as required
4. Attend relevant training sessions, conferences, workshops as required and participate in the personal performance review and development process (PPRD)
5. Undertake other tasks and specific delegated responsibilities associated with the effective operation of the admission and discharge coordination process in consultation with the ward managers
