We are seeking a highly motivated nurse with clinical palliative care knowledge and experience.
Working as part of an integrated team of nurses, covering East Surrey and West Sussex, you will contribute to the overall care of patient and their families referred to St Catherine’s within the community setting, including patients living within care homes. St Catherine’s wish to ensure that the providers of care for people in care homes with palliative and end of life needs have access to support and training to ensure that people are able to die in their preferred place of care, supported by services that are trained to manage their needs. You will also support our growing input to support Frailty needs of patients within our community team. The post holder will be based in Hospice Community Services.
Specialising in the advisory and care coordinator component of this support, you will have shared clinical responsibility for patients across our catchment managed by our Community Manager/Matron.
Our post will give you the opportunity to deliver specialist palliative care to people at end of life. The post holder will be responsible for delivering and managing a small, short-term patient caseload ensuring that effective and robust clinical interventions are implemented. You will be a confident decision-maker and have the passion to always improve and advance. You will contribute to a positive supportive working environment with an emphasis on teamwork and cooperation.
The CNS role at St Catherine’s Hospice aligns with Health Education England’s Advanced Clinical Practice Framework to ensure our underpinning standards are consistent with advanced level practice, including Physical assessment and Prescribing; however, consideration will be given to applicants with relevant experience and demonstrating commitment to acquire these skills within an agreed timeframe.
The post is offered on a full time basis, your working hours will be 37.5 hours per week. You will participate in a rota system providing a 7 day per week cover for our patients and their families, via telephone advice and support, but may include visiting out of hours in exceptional circumstances.

Role: Clinical Nurse Specialist – Care Homes and Frailty
Salary: £38,563.00 - £39,992.00 per annum for 37.5 hours per week, plus weekend enhancements (Grade CA6 A-C) and on-call supplement
Hours: 37.5 hours per week, worked from 8am-6pm on a shift basis over seven days, including participation in an on-call rota. We will also consider part-time hours.
