St Catherine’s Hospice is a local charity with a big ambition: to help everyone face death informed, supported and pain free. Providing expert hospice care, we’re there for people in West Sussex and East Surrey when life comes full circle.
We have an exciting opportunity to join St Catherine's Telephone Advice, Referrals and Rapid Response (STARR) team based in Crawley. STARR is the key single point of contact for patients and families, as well as Health and Social Care Professionals, who require access to services provided by the Hospice. The STARR is also the gateway to receiving, assessing and processing of referrals to the hospice via a single point of access.
Working Monday to Friday as part of the team you will work alongside our team lead, dedicated admin, nurse practitioners and senior staff nurses.
You will be assessing complex patient symptoms and taking appropriate actions, talking to distressed relatives, advising health care professionals, triaging new referrals and with the team lead, oversee our rapid response service who visit patients requiring urgent intervention in the community.
This post is offered on 6 month contract but there may be scope to make this post permanent in the future.
Hours: currently the Nurse practitioners work 3 days 8am to 8pm , but we would be happy to consider other working patterns and shorter days.
