We are seeking enthusiastic Palliative Care Nurse Practitioners to join our new service, St Catherine's Telephone Advice, Referrals and Response (STARR). Both full-time and part-time opportunities available
STARR is a team of nurse practitioners, rapid response nurses and Nursing Assistants who specialise in the provision of triage, symptom advice and support to patients and families facing terminal illnesses.
Working as part of an integrated team of nurses, covering Surrey & West Sussex, you will contribute to the overall care of patient and families referred to St Catherine’s within the community setting. Specialising in the advisory and Care Coordinator component of this support you will have shared clinical responsibility for the patients within one area managed by an Advanced Palliative Care Practitioner.
As a team member you will contribute to a positive supportive working environment with an emphasis on team work and cooperation. You will participate in a rota system providing a 7 day a week face to face and telephone service for patients and their families and, in the out of hours rota providing telephone advice and support. This may include visiting out of hours in exceptional circumstances.
It is anticipated that the work plan will be 20% management, 70% clinical (including a lead expertise) and 10% Education and Teaching
You will participate in a rota covering 7 days a week, the core hours being 8am - 8pm Monday to Friday, participating in a rota and occasional weekends.
