We are delighted to offer this new role of Senior Patient and Carer Coordinator and are looking for a passionate individual who will:
Undertake and facilitate an excellent patient experience across our ward and community services, supporting the patient and family members to navigate our services and signposting to external services when required. This includes support the admissions and discharges of patients to and from the range of clinical services provided to patients at StCH, particularly as they may have complex health and social needs that requires the experience and skills of a large number of professionals from a range of different organisations.
We are looking for an experience individual who has clear knowledge of the discharge processes from a clinical setting. You will be an excellent communicator and have up to-date knowledge of social care and health systems, and have negotiation and influencings skills to handle complex and sensitive situations.
You will work closely with hospice clinicians, community, ward and multi-disciplinary team members (internal and external) to deliver a high quality, patient focussed service.
This post is offered on a full time basis, working 37.5 hours per week, occasional evenings and weekends may be required (part-time/job share will be considered). Applicants will be required to be flexible.

We are located in Malthouse Road, Crawley, West Sussex, which is within commuting distance from Brighton and London and just a 5 minute walk from Crawley railway station.
Role: Senior Patient and Carer Coordinator
Salary: £22,767 per annum (Grade NN4A)
Hours: 37.5 hours per week (part-time/job share considered)
(Monday to Friday - occasional evenings and weekends may be required)
