Planning your Care
Our Advance Care Plan is available to help you plan and share your decisions about future care.
Our team will work with you to encourage you to think about and record how you would like to be cared for in the final months of your life. It’s really helpful to have these conversations while you are still well enough to speak with us. Conversations like this may help you feel more in control and help to reassure you and your family that you will get the care you would like as you become less well.
